Initial Preventative Physical Examination (IPPE)
What is IPPE?
- It is NOT a Head-to-Toe Physical Exam.
- It is a discussion with your provider to identify health risks and develop a plan to correct any risks identified.
- It is an opportunity to take routine measurements and update your medical records.
- The focus of the IPPE is to provide education, counseling and recommended Medicare specific prevention services that are appropriate for the you.
When am I Eligible for IPPE?
New members during the first 12 months of the Medicare effective date located on your Medicare card for part B. This is a once in a lifetime benefit that must be completed prior to the 12th month of enrollment with Medicare.
What if I have had Medicare for more than 12 months?
You may be eligible for the Annual Wellness Visit (AWV).
What is AWV?
- It is NOT a Head-to-Toe Physical Exam.
- The Annual Wellness Visit focuses on a plan to keep you healthy.
What does the IPPE and AWV include?
- Review of Health Risk Assessment form.
- Review the list of doctors that are involved in providing your care.
- Measure blood pressure, height, weight or waist circumference.(If appropriate)
- Detection of cognitive impairment.
- Review of potential risk factors for depression, functional ability and level of safety.
- Written screening schedule (5-10 years).
- List of risk factors and conditions where interventions are recommended.
- Personalized health advice and referrals for health education and preventative counseling.

An advance health care directive, also known as living will, Advance directive, is a set of written instructions that specify what actions should be taken for their health if they are no longer able to make decisions due to illness or incapacity. If you have a completed Advanced Directive please bring it with you so we can discuss the details.

What you need to know about your Appointment:
If you are sick or need to talk to your doctor about a health problem, it cannot be combined with your IPPE or AWV exam. We will address your health issue and a co-payment or co-insurance will apply. Your wellness exam will be rescheduled for a later date.

- Arrive 30 minutes prior to your appointment time.
- Bring your insurance card
